Sliding Fee Discount Program

It is the policy of Family Health Matters Community Health Center to provide essential health care services regardless of a patient’s ability to pay. We offer eligible patients (uninsured and insured) a sliding fee schedule based on income and household size.

Our sliding fee schedule (below) is updated annually using the Federal Poverty Guidelines.  Additional discounts may be available to patients who meet criteria or state or county-funded indigent care programs. Additional discounts are dependent on availability of funds. Patients may apply for a discount at any time.


Low To No Cost Programs

Family PACT is a state program that provides free family planning services to low income men and women. The program’s purpose is to provide the services you need to establish the timing, number of children, spacing of your children and to help you maintain reproductive health.

Every Woman Counts provides free breast and cervical cancer screening and diagnostic services to California’s underserved populations.

 Presumptive Eligibility for Pregnant Women (PE4PW) is a Medi-Cal program designed to provide immediate, temporary coverage for prenatal care to low-income pregnant patients pending a formal Medi-Cal application.

Comprehensive Perinatal Services Program provides enhanced prenatal care integrating healthy eating, psychosocial services and health education.

Child Health and Disability Prevention is a preventive program that delivers periodic health assessments and services to low income children and youth in California. CHDP provides care coordination to assist families with medical appointment scheduling, transportation, and access to diagnostic and treatment services.
